How Does Hospice Care Begin?

Anyone can refer a patient to hospice by calling the admission department for an evaluation. The patient’s primary doctor must give a six-month or less terminal diagnosis. Most referrals come from family members, hospital discharge planners and physicians. Call at (707) 263-6222 / (800) 900-8820 to make a referral and to arrange a home evaluation.

Anyone can make a referral to Hospice by calling the Hospice admission coordinator. Referrals can come from the patient, family member, hospital discharge planners and physicians. An evaluation can only be done with a physician order. The physician must give a six month or less terminal diagnosis and state that the physician, patient and family have discussed palliative care without curative treatment.
