How Is Hospice Care Provided?
Hospice care is provided in the privacy and comfort of your home, or other places of residence such as residential care, assisted living or skilled nursing facilities. Hospice is a team of professionals and volunteers who care for the physical, emotional and spiritual needs of hospice families. Our team consists of:
- Medical director who oversees the plan of care
- Physicians who follow the care of the patient
- RNCM will be assigned and will coordinate the care provided and will make
scheduled home visits to provide skilled care
- Registered nurses who are on call 24 hours a day, seven days a week for consultation or home visits
- Home health aides provide and assist with personal care
- Social workers who provide services to address economic, social and psychological needs
- Spiritual counselors who minister to the spiritual needs of patients and family
- Bereavement counselors who provide emotional and psychological support to those experiencing grief
- Physical therapists who provide assistance with mobility and speech
- Trained volunteers who provide caregiver support
