Social Worker

We are seeking an experienced part–time or per diem  social worker that will be responsible for assessing and addressing psychosocial needs of patients receiving hospice and palliative care through the development of a Plan of Care in concert with other Interdisciplinary Team members.

The social worker participates in the development of individualized patient care plans and facilitates activities related to the social and psychological well-being of patients and their families. This position reports directly to the Director of Patient Care Services.  Minimum qualifications included but are not limited to an MSW from and accredited school, two years of related experience in medical social work an in-depth understanding of hospice philosophy and goals, and excellent communication skills both orally and writing.

Reliable transportation, valid driver’s license, and current vehicle insurance required.
